D=downstroke

Is that how Tim plays it?
Also, when sliding from each little lick , do you leave your index finger on the D/G strings? Or lift it completely?

I can't speak for how Tim plays it exaclty, but this works for me:

For the left hand, I pretty much keep my index and middle fingers on the same strings and move them up the neck.

As for the picking, my preference is to inveret the pickstroke direction as you have it it labeled as I find it easier. By using an upstroke on the A string and a downstroke on the D string, the pick stays in between the two and has to move less to pluck the strings.

One thing Tim does do however, which I find a little more challening, is sometimes he'll just pick all the notes and take out the hammer ons for a different articulation sound.
